剖宫产术后再次妊娠156例分娩方式临床分析

目的:探讨剖宫产术后再次妊娠的分娩方式.方法:对2002年1月至2005年1月我院156例剖宫产术后再次妊娠孕妇分娩方式及母婴并发症进行回顾性分析,将其中剖宫产术后阴道分娩(VBAC)41例与随机抽取非瘢疤子宫阴道分娩(VBNC) 41例进行对照.结果:156例中64例进行阴道试产,41例试产成功,成功率64.1%.选择性剖宫产91例.VBAC组新生儿窒息,产后出血、先兆子宫破裂发生率与对照组比较差异无显著性意义.再次剖宫产(RCS)产后出血量比VBAC组高,平均住院天数比VBAC组长,差异有显著性意义(P＜0.05) .结论:剖宫产史不是再次剖宫产的指征,应结合前次剖宫产的指征、手术方式、术后情况,本次妊娠情况及B超监测伤口情况综合考虑,符合试产条件者,严密监护下可阴道试产.

Clinical Analysis of 156 Cases of Pregnancy Delivery After Cesarean Section

Objective: To analyze pregnancy delivery after cesarean section.Method: 156 cases of pregnant women after cesarean section delivery and again prenatal complications were retrospectively analyzed.which will vaginal delivery after cesarean section(VBAC),and randomly selected 41 cases of uterine scar-grain vaginal delivery(ABNC),41 cases were observed.Result: 64 cases of 156 cases vaginal trial production,the success of the pilot was 41 cases,the success rate of 64.1%.Elective cesarean section 91 cases.There were no significant difference in neonatal asphyxia,postpartum hemorrhage,the incidence of uterine rupture threatened between VBAC and VBNC.Repeat cesarean section(RCS) of postpartum hemorrhage VBAC was higher than average days of hospitalization than VBAC head,there were statistically significant,p<0.05.Conclusion: Another indication of cesarean section is not the history of cesarean section,in combination with the previous indication of caesarean section after operation,the wounds of pregnancy and ultrasound monitoring consideration, with trial production conditions,monitoring vaginal under trial production.
