彩色多普勒超声心动图在小儿先天性肺动脉吊带诊断中的应用

目的探讨彩色多普勒超声心动图在先天性肺动脉吊带诊断中的应用价值。方法以CT血管成像（CTA）和手术证实的12例肺动脉吊带患儿为研究对象，回顾分析其彩色多普勒超声心动图特征。结果12例肺动脉吊带患儿经超声心动图检查确诊10例，漏诊2例。诊断正确率为83％。超声心动图显示肺动脉分叉消失，主肺动脉直接延续为右肺动脉，于右肺动脉发出异常起源的左肺动脉。2例漏诊患儿中1例是肺动脉吊带合并动脉导管未闭，超声心动图仅诊断出动脉导管未闭；另一例为单纯性肺动脉吊带，因超声医师对本病的认识不足，未做出肺动脉吊带的诊断，后行CTA检查，提示肺动脉吊带。结论彩色多普勒超声心动图可在早期对肺动脉吊带作出诊断，常规行彩色多普勒超声心动图检查时，应注意肺动脉分叉处左肺动脉的位置是否异常，多切面检查，减少对本病的漏诊误诊。

Application of color Doppler echocardiograpy in diagnosing congenital pulmonary artery sling

Objective To evaluate the value of color Doppler echocardiography on pulmonary artery sling. Methods The cchocardiography features of 12 patients with pulmonary artery sling confirmed by CT and surgical operations were analysed. Results Among the 12 patients with pulmonary artery sling, 10 were definite eases ,2 were missed diagnosis eases. The aeeuracy rate was 83%. The 10 definite eases showed the disappearance of branch of pulmonary artery, and the left pulmonary artery arose from the right pulmonary artery. The reason for one of missed cases was the combination of pulmonary artery sling and patent duetus arteriosus. Echocardiography only found the patent ductus arteriosus. The other missed case was due to lack of experience of this disease. Conclusion Color Doppler eehocardiography can make diagnosis of pulmonary artery sling at the early phase. More attention should be paid to left pulmonary artery during routine echocardiography examination. The application of echocardiography is of important value in improving the accuracy diagnosis rate of this disease.
