加味补肺汤治疗COPD伴抑郁的临床效果分析

目的 探讨加味补肺汤治疗慢性阻塞性肺疾病(COPD)伴抑郁的疗效,为临床提供参考。 方法 将浙江中医药大学附属第三医院和杭州市周浦社区卫生服务中心2015年1月-2016年6月间收治的256例COPD伴抑郁患者,采用随机数字法分为2组,每组128例。对照组给予常规西医治疗,观察组在此基础上加用加味补肺汤。治疗前后,观察患者中医证候积分、抑郁积分、血清炎症因子、免疫功能的变化,并统计临床疗效。 结果 治疗后,2组中医候状积分各指标均显著降低(P<0.05),且观察组中医证候积分各指标均显著低于对照组(P<0.05)。治疗后,2组HAMA、HAMD均显著降低(P<0.05),且观察组HAMA、HAMD均显著低于对照组(P<0.05)。治疗后,2组TNF-α、IL-6水平均显著降低(P<0.05),且观察组TNF-α、IL-6水平均显著低于对照组(P<0.05)。治疗后,2组IgM、IgG、IgA水平显著升高(P<0.05),且观察组IgM、IgG、IgA水平显著高于对照组(P<0.05)。观察组总有效率为89.8%,显著优于对照组的77.3%(P<0.05)。 结论 在西医常规治疗基础上,加味补肺汤可有效降低COPD伴抑郁患者血清炎症因子水平,提高免疫功能,缓解临床症状。 

Clinical effect analysis of Jiawei Bufei decoction on COPD with depression

Objective To investigate the clinical effect of Jiawei Bufei decoction on chronic obstructive pulmonary disease (COPD) with depression and its influences on TCM syndrome score,depression status,serum inflammatory factors and immune function. Methods Two hundred and fifty-six cases of COPD with depression patients from January,2015 to June,2016 were randomly divided into two groups,with 128 cases in each group.he control group was treated with conventional western medicine,and the observation group was added with Jiawei Bufei decoction.Before and after treatment,the TCM syndromes scores,depression scores,serum inflammatory factors,and immune function of two groups were detected.And the clinical curative effect was accounted. Results After treatment,the TCM syndrome scores of two groups were significantly reduced (P<0.05),and the TCM syndrome scores of observation group were significantly lower than those of the control group (P<0.05).After treatment,HAMA and HAMD of two groups were significantly reduced (P<0.05),and HAMA and HAMD of observation group were significantly lower than those of control group (P<0.05).After treatment,TNF-α and IL-6 of two groups were significantly reduced (P<0.05),and TNF-α and IL-6 of observation group were significantly lower than those of control group (P<0.05).After treatment,IgM,IgG and IgA levels of two groups were significantly increased (P<0.05),and IgM,IgG and IgA levels of observation group were significantly higher than those of control group (P<0.05).The total effective rate of observation group was 89.7%,which was significantly higher than that of control group 77.0%(P<0.05). Conclusion Based on routine western medicine treatment,Jiawei Bufei decoction could effectively reduce the serum inflammatory factors,improve immune function and alleviate the clinical symptoms of COPD with depression patients.
