内镜下钛夹与内镜下药物注射治疗急性非静脉曲张性上消化道出血的临床疗效观察

目的:探讨内镜下钛夹与内镜下药物注射治疗对急性非静脉曲张性上消化道出血(ANVUGIB)患者临床疗效的影响。方法:回顾性分析2018年10月~2019年8月南开医院收治的76例ANVUGIB患者临床资料,根据不同治疗方式将患者分为两组,内镜下药物注射治疗的38例设为对照组,内镜下钛夹治疗的38例设为观察组。比较两组止血效果、炎性因子水平及并发症。结果:观察组即时止血及有效止血率高于对照组,二次出血率低于对照,差异有统计学意义(P<0.05);观察组治疗后IL-6、CRP、TNF-α水平低于对照组,差异有统计学意义(P<0.05);两组并发症无明显差异。结论:内镜下钛夹可有效增强ANVUGIB患者止血效果,降低炎性因子水平,减少二次出血率,安全性高,且效果优于内镜下药物注射治疗。

Endoscopic Titanium Clip and Endoscopic Drug Injection in the Treatment of Acute Non-varicose Upper Gastrointestinal Bleeding

Objective:To explore the effect of endoscopic titanium clip and endoscopic drug injection on the clinical efficacy of patients with acute non-varicose upper gastrointestinal bleeding(ANVUGIB).Methods:The clinical data of 76 patients with ANVUGIB admitted to Nankai Hospital from October 2018 to August 2019 were retrospectively analyzed.The patients were divided into two groups according to different treatment methods.38 patients treated with endoscopic drug injection were set as the control group.Thirty-eight cases of peptide clip treatment under endoscopic were set as observation group.The hemostatic effect,inflammatory factor levels and complications were compared between the two groups.Results:The rate of immediate hemostasis and effective hemostasis in the observation group was higher than that in the control group,and the secondary bleeding rate was lower than that in the control group.In the control group,the difference was statistically significant(P<0.05);there was no significant difference in complications between the two groups.Conclusion:Endoscopic titanium clips can effectively enhance the hemostatic effect of patients with ANVUGIB,reduce the level of inflammatory factors,reduce the rate of secondary bleeding,and have high safety,and the effect is better than endoscopic drug injection treatment.
