肝功能与2型糖尿病周围神经病变的关系

摘    要:目的 探讨2型糖尿病(T2DM)并周围神经病变(DPN)患者的肝功能变化。 方法 分别选择2015年1月-2016年1月在安徽医科大学附属安庆医院接受治疗的2型糖尿病合并DPN(DPN组)和未合并DPN(非DPN组)患者各100例作为观察组,选择同期来我院健康体检患者200例作为对照组,采用生化检验法分别检测血清中丙氨酸转氨酶(ALT)、天冬氨酸转氨酶(AST)和γ谷氨酰转移酶(γ-GT)的含量变化。 结果 DPN组及非DPN组中肝功能异常(ALT和γ-GT浓度升高)的检出率均较对照组显著升高,与非DPN组相比,高水平的ALT和γ-GT在DPN组中发生率更高,而另一个指标(AST浓度升高)出现异常的例数在3组中差异无统计学意义;非DPN组及DPN组患者血清中ALT和γ-GT较对照组水平升高(P<0.05),DPN组较非DPN组患者血清中ALT和γ-GT也明显升高(P<0.05),且DPN组患者随着病情加重,ALT和γ-GT水平呈升高趋势(P<0.05);DPN发病各危险因素多元Logistic回归分析结果显示,ALT和γ-GT是2型糖尿病合并DPN患者的危险因素。 结论 T2DM,DPN与肝功能变化有关,监测并控制ALT和γ-GT水平有利于对DPN进行有效干预并改善其预后。

Relationships between liver function changes and peripheral neuropathy in patients with type 2 diabetes

Abstract:Objective To study the changes of liver function of type 2 diabetes patients with diabetic peripheral neuropathy (DPN). Methods The levels of ALT,AST and γ-GT of 100 cases of T2DM with DPN (DPN group),100 cases of T2DM without DPN (non-DPN group),and 200 healthy control subjects (control group) were tested by biochemical test. Results The cases with elevated ALT and γ-GT both in DPN group and non-DPN group were more than that in the control group,and the rate was more higher in the DPN group as compared with non-DPN group.The difference in the cases with elevated AST was not significant among the three groups.The levels of ALT and γ-GT both in DPN and non-DPN group were higher than that in the control group (P<0.05),while the levels of ALT and γ-GT in DPN group was higher than that in the non-DPN group (P<0.05),with a tendency to rise as the disease got worse.The multiple logistic regression showed that the levels of ALT and γ-GT were the risk factors for T2DM combined with DPN. Conclusion The function of liver is closely related with DPN in patients with T2DM.To monitor and control the levels of ALT and γ-GT will be beneficial for the effective intervention and improvement of the outcome of DPN patients.
