重型颅脑损伤患者血清S100B、IL-6与颅内压力变化的相关性研究

摘    要:目的 检测重型颅脑损伤患者伤后各时间点颅内压的变化及颅内血清中的S100B及IL-6的水平,分析研究血清S100B、IL-6水平与颅内压力变化之间的关系。 方法 选取2012年1月-2016年4月于嘉兴学院附属第二医院就诊的重型颅脑损伤患者共88例作为本次研究的研究对象。对患者的颅内压进行连续性的监测,并于入院后的6、12、24、48和72 h分别采集患者的静脉血。检测血清中的S100B蛋白及IL-6水平,采用酶联免疫吸附法。分析不同时间点血清IL-6及S100B水平,并对IL-6及S100B水平变化与颅内压的水平进行相关性分析。采用SPSS 17.0统计学软件进行分析,组间比较采用t检验,计量资料的相关性分析采用Spearman相关性分析,P<0.05为差异具有统计学意义。 结果 重型颅脑损伤患者的S100B水平在受伤后逐渐升高,24 h时达到峰值,随后便逐渐下降;而患者的IL-6和颅内压水平在受伤后逐渐升高,患者血清IL-6及S-100B水平与颅内压水平存在显著相关性(r=1.933、r=4.859,P<0.05)。但是S100B的峰值出现的比颅内压早,而IL-6的变化趋势与颅内压的变化趋势相符。 结论 重型颅脑损伤后的颅内压力变化与血清中的S100B和IL-6水平呈正比,因此,S100B和IL-6水平可以反映患者的损伤程度,为患者的病情判断提供临床依据。

Correlation between serum S100B and IL-6 levels and intracranial pressure in patients with severe craniocerebral injury

Abstract:Objective To detect the changes of intracranial pressure and the level of S100B and IL-6 in the serum of the patients with severe brain injury after injury,and to analyze the relationship between the changes of serum S100B and IL-6 levels and intracranial pressure. Methods A total of 88 patients with severe brain injury in our hospital from January,2012 to April,2016 were selected as the research subjects.The intracranial pressure of patients was monitored continuously.The venous blood of patients was prepared at 6,12,24,48 and 72 h after the admission.The serum S100B protein and IL-6 levels at different time points were tested by enzyme-linked immunosorbent assay.The relationship between the changes of serum S100B and IL-6 levels and intracranial pressure was analyzed statistically.The t test was performed to compare the means of two groups.Spearman correlation analysis was performed for pairs of quantitative data.When P<0.05,the difference was statistically significant. Results The level of S100B in patients with severe craniocerebral injury increased gradually after the operation,and reached the peak within 24 hours,then decreased gradually;while patients with IL-6 and intracranial pressure after injury gradually increased,serum IL-6 and S-100B levels were significantly associated with intracranial pressure (r=1.933,r=4.859,P<0.05).But the peak value of S100B appeared earlier than that of intracranial pressure;however,the trend of IL-6 was consistent with the change of intracranial pressure. Conclusion The changes of intracranial pressure in patients with severe head injury is proportional to the serum S100B and IL-6 levels.Thus S100B and IL-6 levels can reflect the degree of injury and provide clinical basis for evaluation of patients.
