关节镜治疗膝关节盘状半月板损伤的探讨

目的 探讨关节镜治疗盘状半月板的疗效。方法 58例盘状半月板患者，其中不全型20例，完全型29例，Wisberg韧带型9例，根据分型与损伤情况行盘状半月板完全切除与部分切除。结果 本组58例，随访1～6年，平均4年1月，优26例，良24例，可5例，差3例，优良率为86．2％。完全型盘状半月板中，全切患者的优良率为87．5％，部分切除成形术优良率为46．2％，二者有显著性差异；不完全型盘状半月板中，全切患者的优良率为77．8％，部分切除成形术优良率为81．8％，两者无显著性差别。结论 关节镜下盘状半月板成形术可获得优良疗效；完全型盘状半月板主张盘状半月板完全切除，而不全型主张尽可能行关节镜下盘状半月板部分切除成形术。

Methods Fifty-eight cases of discoid menisci were underwent arthroscopic lateral partial or total menlscectomy in the light of type and injury condition .Results Bases on Ikeuchi's grading,26 cases were excellent ,24 cases good and 5 cases fair, the excellent and good result was 86.2%. In cases of complete type of discoid meniscus, the excellent and good result was 87. 5% which patients treated with total meniscectomy, the excellent and good result was 46.3% which patients treated with partial meniscectomy, the difference was significant . In cases of incomplete type of discoid meniscus, there were no significant difference between total and partial meniscectomy. Conclusions Arthroscopic management for discoid meniscus can obtain excellent and good efficacy .
