改良Moberg皮瓣在指端缺损修复中的应用

目的运用改良Moberg皮瓣修复指端软组织缺损，避免术后发生末节指背侧皮肤坏死并获得良好的指腹外形方法首先将一侧的指固有神经血管束包纳入皮瓣中，以保证对皮瓣的血供，保留另一侧的血管束在原指位，以保证术指血供的完整其次，相向缝合创面远端两边残三角形皮肤瓣，增加推进皮瓣的绝对长度，减少整个指掌侧皮瓣的剥离范围。结果本组9例，在手术结束时已确定皮瓣和手指血供的完整，随访无1例坏死，再造指腹外形满意。结论同时二处改良Moberg皮瓣是可靠、有效的修复指尖和指腹的方法。

Use of a modified Moberg flap for repair of fingertip and pulp defect

Objective To introduce the application modified Mobergs flap to avoid ischemic necrosis of the dorsal skin of the distal finger and to obtain cosmetic contour of the terminal pulp after operation. Methods First, one of the neurovascular bundles was included in the flap to supply the flap and another bundle was left in the original position to supply the finger. Second, the residual skin flaps of the distal injured surface were sutured together, not only to increase length of the flap but also to reduce dissected distance of the digital volar advancement flap. Results In 9 cases, the blood supply of the flap and the finger were intact after operation no necrosis was founded at follow-up, and the reface of the terminal pulp was satisfactory. Conclusion The modified Moberg flap in two sites at the same time is a new,available and reliable method for repair of the finger tip and pulp defect.
