阴道超声监测宫颈形态在预测早产及诊治中的临床研究

目的：研究阴道超声监测宫颈形态在早产的预测方面的可行性及价值。方法：选用2009年1月至2010年5月我院收治的有先兆早产症的孕妇60例为研究组，同期在门诊产检的正常单胎初产妇70例为对照组，观察两组在宫颈长度、宫颈指数、内口形态方面预测早产的特异性和敏感性，同时观察治疗前后研究组的自身对照情况。结果：①研究组宫颈长度（2．56±0．62）cm，明显短于对照组正常孕妇的宫颈长度（3．74±0．71），差异有明显统计学意义（P〈0．001）；②研究组宫颈指数（0．68±0．21），明显高于正常孕妇的宫颈指数（0．35±0．23），差异有显著性（P〈0．001）；③研究组宫颈、内口漏斗型17例，其中发生早产14例（占82．35％），正常组宫颈均成“T”型；④研究组中经治疗后症状消失者46例，2周后复查宫颈长度有所增加，平均增加0．56cm。结论：阴道超声监测宫颈形态可作为早产预测的客观指标，也可作为治疗早产疗效判断的可靠指标，值得临床医师重视。

Objective：Studies the vagina supersonic monitor cervix of the uterus shape at premature delivery＇s forecast aspect feasibility and the value. Result：（1）Study team cervix of the uterus length （2.56 ± 0.62） cm, obvious short in control group normal pregnant woman＇s cervix of the uterus length （3.74±0.71）, the difference has the obvious statistics significance （p 〈 0. 001）;（2）The study team cervix of the uterus index （0.68±0.21）, is higher than the normal pregnant woman＇s cervix of the uterus index obviously （0.35±0. 2：3）, the difference has the significance （p 〈 0. 001） ;（3）Study team cervix of the uterus boring funnel 17 examples, occurs prematurely delivers 14 examples （to account for 82.35%）, the normal group cervix of the uterus becomes ＂T＂;（4）In study team after treatment symptom vanish 46 examples, 2 weeks later reexamines the cervix of the uterus length to increase, increases 0.56era equally. Conclusion：The vagina supersonic monitor cervix of the uterus shape may the objective indicator which forecast as the premature delivery, may also take the treatment premature delivery curative effect judgment the reliable target, is worth clinician taking seriously.
