皮下通道胆囊肝胆管成形术治疗肝胆管结石和胆管狭窄的中远期疗效

目的　总结皮下通道型胆囊肝胆管成形术治疗肝胆管结石及胆道狭窄的中远期并发症。方法　统计 1994年 12月至 2 0 0 2年 3月我科 87例行STHG手术患者的术后中远期并发症。结果　STHG并发症发生率较低 ,而且并发症的种类也较少 ;STHG组基本无返流性胆管炎的表现 ,而且无胃肠道功能紊乱和吻合口溃疡发生。结论　该术式既保存了胆囊及Oddi括约肌功能 ,又保证了胆汁的生理流向 ;防止了肠液的返流 ,避免了消化功能紊乱 ,防止了返流性胆管炎 ,对患者的影响小 ,是一种较为理想的治疗肝胆管结石和肝门部胆管狭窄的术式。

Clinical Effects of Subcutaneous Tunnel and Hepatocholangioplasty with the Use of Gallbladder in the Treatment of Hepatocholelithiasis and Biliary Stenosis

Objective To summarize the middle-term and long-term complications of STHG in the treatment of hepatocholelithiasis and biliary stenosis.Methods To analysis the complications of STHG operation which performed during Dec. of 1994 to Mar. of 2002 in 87 patients.Results STHG group had low complication incidence,and also had only a few kinds of complications,especially had no occurance of reflux cholangitis,disorder of intestinal function and ulcer of anastomosis orifice,ect.Conclusions This operation not only maintains the physiological function of gallbladder and Oddi sphincter,but also keeps the physiological way of bile fluid and prevents intestinal juice reflux.In this way,STHG can avoid dysfunction of GI tract and occurrence of reflux cholangitis.So STHG is a novel and more suitable operation in deal with hepatocholelithiasis and biliary stenosis.
