止血带加压性疼痛导致交感神经兴奋性的改变

摘    要:目的:研究止血带加压性疼痛是否导致了交感神经兴奋性的改变并参与双手皮肤血流量的调节.方法:20名健康成年男性受试者,实验前填写相应心理学量表,然后进行3次左上臂止血带加压,连续记录受试者的疼痛强度和不适度,同时用红外热像仪采集双手皮肤的温度.结果:加压性疼痛阈值逐渐降低[F(2,57)=128.36,P<0.001],平均疼痛强度F[(2,57)=7.78,P<0.01]和不适度[F(2,57)=5.16,P<0.05]均逐渐降低.双手指温度差异值比较差异显著[F(1,19)=479.08,P<0.001],3次加压之间差异显著[F(2,19)=86.63,P<0.001];双手掌温度差异值比较差异显著[F(1,19)=126.08,P<0.001],3次加压之间差异显著[F(2,19)=110.76,P<0.001].3次比较,左手掌平均温度先升高后降低,右手掌、左右手指平均温度逐渐增高.结论:在止血带加压过程中,疼痛导致交感神经兴奋性的改变,参与了双手皮肤血流量的调节.

Abstract:Objective:To investigate the changes of pain sensitivity and temperature of two hands following conditioning of pneumatic tourniquet tests and to explore the possible mechanism of pain-related sympathetic activities from infrared thermal imaging.Methods:20 healthy male subjects (age:24±2.95 yrs.) participated in the study.They were asked to fill in the informed consent and a series of psychological questionnaires before experiment.Cuff-pressure pain was induced at the left upper arm and each subject went through the tonic cuff pressure test for three separate times.Pain intensity and distress were recorded and the temperature of two hands was also acquired with medical infrared thermal imaging.Resuits:Compared to the first session,the pressure-pain threshold was lower in the second session and lowest in the third session [ F(2,57) = 128.36,P <0.001 ].The average pain intensity and average distress decreased in the second and third session [ intensity:F(2,57) =7.78 ,P <0.01;distress:F(2,57) =5.16 ,P <0.05 ].The hand temperature was significantly different between left palm and right palm [F(1,19) = 126.08,P <0.001 ].The temperature was also significantly different between left fingers and right fingers [F(1,19) = 479.08,P <0.001 ].Evidently,the temperature of the contralateral right palm and fingers became higher and higher with the sequential sessions.Conclusion:Pressurized tourniquet pain may cause changes in sympathetic activity as inferred from skin temperature dynamics.
