腹内胃肠道外间质瘤的CT诊断

目的分析腹腔内胃肠道外间质瘤的CT特点．旨在提高对该病的CT诊断水平。方法回顾性分析17例22个经手术病理证实的腹腔内胃肠道外是质瘤的平扫及增强CT表现。结果（1）肿块位于胃小弯区2个，中腹部肠系膜区4个，右侧肠系膜区1个，盆腔2个，腹腔内巨大占据多部位7个，腹膜后区4个，腹壁2个。（2）分叶状14个，不规则形5个，类圆形3个；病变大部分有塑形生长。（3）19（19／22）个肿块内密度不均匀，内有低密度坏死区，直径均〉10cm。3（3／22）个肿块呈均匀软组织密度，直径≤10cm。20（20／22）个肿块内无液气面。（4）增强21个肿块明显不均匀强化，1个肿块呈中度均匀强化。结论胃肠道外间质瘤大多体积较大，坏死多见，呈明显不均匀强化，多不与胃肠道相穿通。CT表现有相对的特征性，结合CT影像特点和临床资料综合分析可提高诊断率。

Diagnosis of Extragastrointestinal Stromal Tumors with Computed Tomography

Objective To study the CT features of extragastrointestinal stromal tumors for improving the CT diagnosis level. Methods Both plain and enhanced CT findings of 22 leisions of 17 cases of cxtragastrointestinal stromal tumors were studied retrospectively, these cases were proved by surgical pathology.Result （1） Tumors originated from the gastro lesser curvature（n=2）, mid abdominal mesentery（n=4）,right mesentery（n= 1 ）, cavitas pelvis（n=2）, abdominal cavity multilocation（n=7）, retroperitoneum（n=4） and abdominal waU（n=2）.（2） Contours of the tumor were lobulated （n=14） and irregular（n=5） and round （n=3） .The growth trend of most tumors have plasticity.（3）These tumors showed uneven density and necrosis（n=19） and even density（n=3）. The former diametes exceed 10 cm and the latter equal or below 10 cm. These tumors were free liquid-gas surface（n=22）. （4） On enhanced CT, these tumors showed obvious heterogeneous enhancement（n=21） and mid even enhancement（n=1）. Conclusion Most extragastrointestinal stromal tumors show fairly large capacity,necrosis more common,markedly uneven enhancing and not perforation with the gastro-intestinal tract.CT findings of extragastrointestinal stromal tumors have some characteristics.To combine the imaging and pathologic features can improve the diagnostic rate of extvagastrointestinal stromal tumors.
