高海拔地区加压空心钉治疗新鲜老年股骨颈骨折术后骨不愈合与股骨头坏死影响因素的研究

摘    要:目的:探讨高海拔地区通过加压空心钉治疗老年新鲜股骨颈骨折术后骨不愈合与股骨头坏死的影响因素.方法:对2000年3月-2009年1月收治的112例新鲜老年股骨颈骨折行加压空心钉内固定手术治疗且资料完整的患者进行回顾性分析,对年龄、性别、骨折Garden分型、受伤至手术时间、复位质量等因素进行分析.结果:全部患者获得随访,随访时间平均为30.8个月,11例发生骨不愈合,发生率9.82%;17例发生股骨头坏死,总坏死率15.18%.结论:骨折Garden分型、骨折复位质量、年龄是老年股骨颈骨不愈合及股骨头坏死的主要影响因素,而伤后2周内手术骨不愈合及股骨头坏死率无差异,性别对骨不愈合及股骨头坏死无影响.高海拔地区缺氧环境对骨不愈合及股骨头坏死无明显影响.

Abstract:Objective:To discuss the factors of influenced bone nonunion and femoral head necrosis after treatment of fresh femur neck fracture with compress cannulatedscrews in older at high altitude.Method:A total of 112 patients with fresh femoral neck fracture and underwent internal fixationsurgery with compress cannulatedscrews were collected from March,2000 through Jan,2009 in our hospital;their data including age,gender,fracture garden types,time from injury to operation,quality of treatment were retrospectively reviewed.Results:All patients received 30.8 months of follow up,of 112 cases,11caseshad nonunion(9.82%) and 17cases for femora head necrosis(15.18%).Conclusions:The Garden classification of fractures、the treamed quality and age are the main influential factors resulted from femoral head necrosis and bone nonunion after treatment of femoral neck fracture with compress cannulatedscrews.There is no relativity in injury time in two weeks and gender for nonunion and femoral head necrosis.Effect of high-altitude environment on nonunion and femoral head necrosis could not available.
