磺胺甲噁唑在不同居住时间、不同海拔高度汉族、藏族健康者体内与蛋白和红细胞的结合率

摘    要:目的 比较磺胺甲噁唑在久居平原、急进高原、久居高原、世居高原汉族和藏族健康者体内与蛋白、红细胞的结合率.方法 取健康男性志愿者口服磺胺甲噁唑后1.5、12.0和48.0 h时间点血样.超率法测定蛋白结合率;红细胞结合率通过红细胞压积、血浆和全血药物浓度计算.药物浓度均采用反相高效液相色谱法测定.结果 久居平原、急进高原、久居高原、世居高原汉族和藏族健康者体内的蛋白结合率分别为65.24%、77.50%、71.33%、67.33%和70.47%,红细胞结合率分别为6.04%、6.90%、9.24%、7.39%和8.29%.与平原组比较,急进高原、久居高原和世居高原藏族健康人体内磺胺甲噁唑的蛋白结合率均显著升高.久居高原、世居高原汉族和藏族健康人体内磺胺甲噁唑的红细胞结合率显著高于平原.结论 高原低氧使健康人体内磺胺甲噁唑的蛋白和红细胞结合率均显著升高.

Abstract:Objective To compare the protein and red blood cells(RBC) binding rate of sulfamethoxazole in healty volunteers living at plain,after acute and chronic exposure to high altitude,and native Han and Tibetan volunteers living at high altitude.Methods Sulfamethoxazole tablets was orally administrated at 1.5h,12h and 48 h after volunteers living at plain and high altitude and then the blood sample was collected in order to obtain whole blood and plasma samples.sulfamethoxazole-Protein binding rate was determined by ultrafiltration method,and the sulfamethoxazole-RBC binding rate was calculated with hematocrit,whole blood and plasma concentration.The concentration of sulfamethoxazole in biological sample was determined by RP-HPLC method.Results The binding rate of plasma protein with sulfamethoxazole in vivo was 65.24%,77.50%,71.33%,67.33% and 70.47% in the plain,acute exposure,chronic exposure,native Han and native Tibetan groups,respectively.The binding rate of RBC with sulfamethoxazole was 6.04%,6.90%,9.24%,7.39% and 8.29% in the plain,acute exposure,chronic exposure,native Han and native Tibetan groups,respectively.The binding rate of plasma protein with sulfamethoxazole was 18.8%,9.3% and 8.0% higher in acute exposure,chronic exposure and native Tibetan groups,respectively,compared with the plain group.The binding rate of RBC with sulfamethoxazole was 53.0%,22.4% and 37.3% higher in chronic exposure,native Han and native Tibetan groups,respectively,compared with the plain group.Conclusion The effect of high altitude hypoxia on the protein and RBC binding rate of sulfamethoxazole in healthy volunteers is significant.
