摘    要:目的 研究持续声门下吸引在ICU预防机械通气(mechanical ventilation,MV)患者呼吸机相关性肺炎(ventilator-associated pneumonia,VAP)中的作用.方法 根据是否行持续声门下吸引将入住ICU行MV超过48 h患者分为观察组(持续声门下吸引组)和对照组(常规人工气道组),分析2组患者VAP发生率、呼吸机使用时间、ICU平均住院时间.结果 观察组和对照组VAP发生率分别为28.12%和48.39%,呼吸机使用时间分别为6.9 d和9.7 d,平均住院时间分别为15.9 d和22.6 d;差异均有统计学意义(P<0.05).结论 应用有效的持续声门下吸引可降低VAP发生率,缩短MV治疗时间,减少ICU平均住院时间.

Abstract:Objective To research the effects of continuous aspiration of subglottic secretion for preventing ventilator-associated pneumonia(VAP) in the patients with mechanical ventilation in ICU.Methods According to whether or not using continuous aspiration of subglottic secretion,the patients with mechanical ventilation(MV)>48 h in ICU of our hospital were divided into the observation group(continuous aspiration of subglottic secretion) and the control group(routine artificial airway).The incidence rate of ventilator-associated pneumonia(VAP),use time of respirator and average stay duration in ICU were analyzed between the two groups.Results The incidence rates of VAP in the observation group and the control group were 28.12% and 48.39% respectively,the ventilator time was 6.9 d and 9.7 d,the average hospitalization time was 15.9 d and 22.6 d,showing statistical differences between the two groups(P<0.05).Conclusion Application of effective continuous aspiration of subglottic secretion could reduce the incidence of VAP,shorten the time of MV treatment and reduce the average stay duration in ICU.
