奥氮平治疗老年精神分裂症的效果及其安全性分析

目的:探讨奥氮平治疗老年精神分裂症患者的临床效果及其安全性。方法:选取2012年1月至2015年6月我院诊治的69例老年精神分裂症患者,根据就诊单双号分为奥氮平组35例、利培酮组34例,医师指导下连续用药8周后对比两组临床疗效及安全性指标。结果:治疗前、治疗4周、8周的两组阳性与阴性症状量表(PANSS)总分差异均不具有统计学意义(P0.05),治疗4周、8周,两组患者的PANSS总分较治疗前均显著的降低(t=-8.692,-12.667,-8.336,-11.676;P0.001);治疗8周,奥氮平组的TC、LDL-C、血清瘦素、FPG高于利培酮组(t=3.386,2.954,3.256,6.800;P0.05)、HDL-C低于利培酮组(t=-5.637,P0.001);治疗过程中,利培酮组的心电图指标变化率32.35%高于奥氮平组11.42%(χ~2=4.438,P=0.035)。结论:奥氮平治疗老年精神分裂症效果可靠,对患者血脂、血糖、瘦素水平具有一定程度影响,心脏不良反应发生率较低。

Effect and Safety of Olanzapine in the Treatment of Elderly Patients with Schizophrenia

Objective:To study the clinical effect and safety of olanzapine in the treatment of elderly patients with schizophrenia.Methods:From January 2012 to June 2015 in our hospital,69 elderly patients with schizophrenia were divided into olanzapine group and risperidone group and 35 cases in 34 cases,after 8 weeks clinical curative effect and safety index were compared between two groups under the guidance of doctors.Results:Before treatment,at 4 weeks and 8 weeks of treatment,the scores of PANSS scale of two groups were not statistically significant differences,after treatment for 4 weeks and 8 weeks,the PANSS scores of the patients in the two groups were significantly lower than those before treatment(t =-8.692,-12.667,-8.336,-11.676;P ＜0.001);After treatment,TC,LDL-C,serum leptin and FPG in olanzapine group were higher than those in the risperidone group (t =3.386,2.954,3.256,6.800;P ＜ 0.05),HDL-C was lower(t =-5.637,P ＜0.001);In the course of treatment,changes of electrocardiogram in risperidone group was higher than that in olanzapine group(x2 =4.438,P =0.035).Conclusion:Olanzapine in the treatment of senile schizophrenia effect is reliable,has a certain degree of influence on eptin levels in patients with blood lipid,blood glucose,and a low incidence of adverse reactions of heart.
