纤维支气管镜代胸腔镜术在肺癌分期中的应用价值

目的：评价纤维支气管镜代胸腔镜术在肺癌分期中的应用价值。方法：选择47例原发性肺癌伴胸水患者，取患侧（胸腔积液一侧）腋中线或腋后线第6或7肋间为切口，消毒及局部麻醉后，切开约15mm，置入不绣钢套管（长10cm）troear，在纤维支气管镜下进行胸膜探查、活检。结果：47例原发性肺癌伴胸水患者在纤维支气管镜下行胸膜活检，29例有胸膜转移，3例纵隔转移，15例无胸膜转移。结论：纤维支气管镜代胸腔镜术有效提高肺癌的精确分期，避免不必要的开腔探查，还进一步提高了肺癌手术切除率。

Value of substitution of thoracoscopy with bronchofibroscope in the staging of lung cancer

Objective To appraise the value of bronchofibroscope taking place of thoracoscopy in the staging of lung cancer.Method Total 47 cases of primary lung cancer accompany with pleural fluid were chosen and the incision was located in the 6th or 7th intercostal space on midaxillary line or posterior axillary line in the sick side.After disinfection and local anesthesia,the incision was about 15mm,and the 10 mm trocar was inserted.Pleural explore and biopsy was made under bronchofibroscope.Results Among 47 patients with primary lung cancer accompany with pleural fluid received biopsy under bronchofibroscope,29 had pleural metastasis,3 with mediastinum metastasis,and 15 without pleural metastasis.Conclusion The substitution of thoracoscopy with bronchofibroscope can elevate the precise staging of lung cancer effectively,avoid unnecessary open explore,and elevate the excising rate of lung cancer.
