钛质人工听骨在鼓室成形术听力重建中的应用

摘    要:目的:探究采用钛质人工听骨进行鼓室成形术听力重建的疗效。方法:对38例不同分型的慢性中耳炎患者采用单纯鼓室成形、联合进路鼓室成形及开放性鼓室成形三种术式,以钛质人工听骨行听骨链重建,并进行为期13~47个月的随访。通过计算术前术后纯音测听测试气导骨的平均值,比较手术前后气骨导差。结果:38例鼓膜愈合良好,基本平坦,1例约术后13个月出现小穿孔,2例鼓膜轻度内陷,活动度较差,但无粘连。临床观察均未发现中耳炎及胆脂瘤复发,未见人工听骨排异反应。行纯音测听,术前气导听力与术后比较,差异具有统计学意义(P〈0.05);术前骨气导差与术后比较,差异具有统计学意义(P〈0.05)。结论:鼓室成形术使用的钛质人工听骨,生物相容性好,组织亲和性佳,能明显改善患耳的听力,提高患者的生活质量,值得推广。

Abstract:Objective: To explore the therapeutic effects of titanium prostheses in tympanoplasty. Methods: Thirty-eight patients (thirty-eight ears) aged between 18 to 67years old who were diagnosed as chronic otitis media of different types underwent pure tympanoplasty, bi-path tympanoplasty and open tympanoplasty respectively. After that the ossicular chain was reconstructed by titanium prosthesis. Follow-up of 13-47 months were carried out. Bone-conducting hearing levels and air-conducting hearing levels through pure tone audiometry test were compared. Results. The membrane tympani of all ears have been well healed. No relapse of cholosteatoma or transplant rejection was found. The air-conducting hearing levels of the average language Frequency after undergoing the operations were significantly improved (P〈 0.05). And there was significant difference in the gap between bone-conducting hearing levels and air-conducting hearing levels beore and after undergoing the operation. Conclusions.. The new material-titanium, showing high biocompatibility with human body tissues, which could improve the average hearing level and the patients' quality of life, it's worthy of application in large scale.
