侧脑室-腹腔分流术治疗脑积水109例临床观察

目的：探讨侧脑室-腹腔分流术治疗脑积水的临床疗效及术后并发症的处理。方法：109例脑积水患者采用侧脑室-腹腔分流术治疗。结果：109例脑积水患者中，术后症状明显改善者93例。好转者14例，症状元改善者2例。8例发生术后并发症，其中分流管阻塞5例，术后感染1例，精神障碍1例，癫痫1例。结论：侧脑室-腹腔分流术治疗脑积水疗效显著，术中严格无菌操作，减少手术的创伤及采用最佳的置管方式是减少手术并发症、提高手术成功率的关键。

Cerebrospinal Fluid Shunting Surgery for Hydrocephalus

Objective:To explore the shunting surgery for hydrocephalus and its common complications. Methods:113 ventriculoperitoneal shunts and 1 ventriculoatrial shunt for 109 patients with hydrocephalus were performed in our department from January,2001 to October,2004. Results: 107 cases had a good result. Postoperative complications were found in 8 cases, including shunt apparatus blockage(5 cases),shunt infection(1 case),mental aberration(1 case), and epilepsy(1 case).They improved after the corresponding measures were taken. Conclusion: Ventriculoperitoneal shunt is the most common shunt style for hydrocephalus, but sometimes ventriculoatrial shunt is also the optimal choice for some patients unsuitable for ventriculoperitoneal shunt. The shunt apparatus blockage and infection are common postoperative complications. Intraoperative aseptic technique, the minimally invasive procedure, and the optimal placement of shunt tube may play an important role in improving the outcome of cerebrospinal fluid shunting surgery for hydrocephalus.
