鼻饲温开水抢救糖尿病脑梗死后高渗综合征

目的探讨鼻饲温开水对糖尿病脑梗死后合并高渗综合征时血Na+、血糖、有效渗透压等的影响。方法观察对象随机分为鼻饲组与对照组。鼻饲组采用胃管注入温开水加静脉补液,对照组单纯静脉补液。结果鼻饲组28例中25例(89.3％)、对照组15例中7例(46.7％)血浆有效渗透压降至320 mmol/L以下,2组比较有显著性差异(P<0.01);鼻饲组显效25例(89.3％),死亡3例(10.7％);对照组显效7例(46.7％),死亡8例(53.3％),2组疗效比较有非常显著性差异(P<0.01) 结论 鼻饲温开水抢救糖尿病脑梗死后合并高渗综合征早期血糖维持在(13.5±5.50)mmol/L,血浆有效渗透压维持在(310±12.9)ml/L可显著降低死亡率。

Intubation feeding of warm water in saving diabetic cerebral infarction complicated by hypertonic syndrome

Objective To investigate the effect of intubation feeding of warm water on blood Na+ , blood sugar and effective osmotic pressure in diabetic cerebral infarction complicated by hypertonic syndrome. Methods The patients were randomly divided into intubation feeding group and control group. The former was treated by intubation feeding of warm water and intravenous injection, the latter was treated by simple intravenous injection. Results Among 28 cases in intubation feeding group, effective osmotic pressure in 25 cases were reduced to less than 320 mmol/L,25 cases were cured and 3 cases died. Among 15 cases in control group, effective osmotic pressure in 7 cases were reduced to less than 320 mmol/L,7 cases were cured and 8 cases died. There were significant differences between the two groups ( P <0.01) .Conclusion Using intubation feeding of warm water in saving diabetic cerebral infarction complicated by hypertonic syndrome, patients' death rate can be greatly reduced if the early levels of blood sugar and the effective osmotic pressure are kept at (13.5 ± 5.50)mmol/L and (310±12.9)mmol/L respectively.
