透明质酸钠治疗膝骨关节炎的临床分析

目的比较透明质酸钠关节内注射治疗膝骨关节炎两种治疗剂量的疗效。方法246例膝关节骨性关节炎患者,随机分为两组,均给予关节内注射透明质酸钠,A组每周1次,5次为1个疗程B组隔周1次,3次为1个疗程。治疗时间均为5周,分别在治疗前、治疗结束时、治疗结束后3个月和6个月进行临床评价。结果A组Lysholm膝关节评分从治疗前平均48.25分上升至88.74、84.27和79.67分。而B组从治疗前平均50.82分上升至89.48、82.11和76.50分。A组在治疗后各时段的有效率分别为95.06%、85.25%和79.51%;而B组为91.94%、87.10%和69.35%。B组有效率在治疗后6个月后明显低于A组。将两组分为轻、重症两个亚组,比较4组的有效率,结果显示治疗后6个月后重症B组的有效率明显低于其他3组。结论透明质酸钠关节内注射是治疗膝骨关节炎的有效治疗方法,建议对于轻症患者每个疗程只需给予3次注射,而重症患者每个疗程应给予5次注射。

Clinical study on knee osteoarthritis treated by hyaluronate

Objective To compare the effect of treating knee osteoarthritis by injecting hyaluronate with two different dosages. Methods Two hundred and forty-six cases with knee osteoarthritis were randomly classified into two groups. The patients in group A were given one injection of hyaluronate every week during 5 weeks, while the patients in group B were given one injection every 2 weeks, so totally 3 injections within 5 weeks. All of the cases received clinical evaluation when pre-treatment, posttreatment, 3 months after treatment and 6 months after treatment. Results The mean Lysholm knee score in group A was 48.25 before treatment, while the score increased to 88.74,84.27 and 79.67 after treatment. The mean score in group B was 50.82, increasing to 89.48, 82.11 and 76.50. The effective rates in group A were 95.06%, 85.25% and 79.51% respectively. The effect rates in group B were 91.94%, 87.10% and 69.35%. The effective rate in group B was significantly lower than in group A when 6 months after treatment. Then,both two groups were classified into two subgroups (mild group and severe group) according to the Lysholm knee scores (mean score was 29). The effective rate of each group was analysed again. The data showed that the effective rate in severe group B was significantly lower than in the other 3 groups. Conclusion The injection of hyaluronate is effective in treating knee osteoarthritis.The authors recommend that 3 injections for mild patients and 5 injections for severe patients.
