成人胡桃夹综合征介入诊治(附4例报告)

目的探讨成人胡桃夹综合征的临床特点及介入诊治的价值方法4例患者19～39岁,均在外院误诊误治,主要表现为下腹持续疼痛、经量增多、经期延长或反复肉眼血尿B超、CTA及MRA示肾静脉远端扩张或盆腔静脉曲张3例行左肾静脉内放置自膨式支架,1例行左肾静脉重建术结合文献讨论其发病、临床及影像学特点和介入诊治的价值结果术后症状消失,3月及6月分别复查B超和CT示左肾静脉通畅,尿常规正常结论左肾静脉压迫综合征具有一定的临床特点,介入内支架治疗可解除左肾静脉受压

Interventional Therapy of Nutcracker Syndrome: A Report of 4 Cases

Objective To study the clinical features and the value of interventional treatment of nutcracker syndrome.Methods Four adult patients with nutcracker phenomenon were reported.They had been misdiagnosed before our treatment.The clinical features were characterized by recurrent episodes of gross hematuria,hypogastralgia,change of menstrual volume and so on.B-mode ultrasound,CT angiography and MRA showed that the left renal vein was dilated and pelvic varix in all the 4 patients.For treatment,left renal intravenous stents set up in 3 cases and reconstruction of left renal vein was performed in one case.The characteristics of onset,imaging,and interventional treatment of left renal vein entrapment syndrome were reviewed and discussed with review of literature.Results These patients were cured and discharged.The clinical symptoma were improved after treatment and follow-up for 3 and 6 months,CT and ultrasonography showed that these renal veins out flow being normal.Conclusion Nutcracker syndrome is clinically of certain characteristics,the endovascular stent placement is effective diagnostic and therapeutic method.
