动态三维超声心动图在二尖瓣修复术中的应用

摘    要:目的评价动态三维超声心动图在二尖瓣修复术中的作用。方法对15例二尖瓣脱垂行修复术的病人进行动态三维重建,并作术前和术后对照。结果术前三维重建能够清晰显示二尖瓣脱垂的部位、范围和瓣下结构的改变情况;术后三维重建能够显示瓣膜修复术后的形态改变,评价修复术的效果,同时可见软质瓣环不会改变二尖瓣环原有的生理特性,对左心基底部的活动影响较小。结论动态三维超声重建技术能够用于二尖瓣修复术术前挑选病人、术后及时观察效果,本研究为人工二尖瓣的合理设计提供依据。

Abstract:Purpose To evaluate the role of three-dimensional echocardiography in mitral valve repair. Methods Fifteen patients with mitral valve prolapse undergone mitral valve repair and annuloplasty were reconstructed before and after operation. Results The extent and severity of mitral prolapse could be directly visualized from the top of the left atrium. The three-dimensional reconstruction also permitted a variety of image orientation of mitral valve apparatus to optimize the visualization of its surgical pathology. The prosthetic mitral annulus ring could be demonstrated clearly in all patients including the flexible and rigid ring. The motion of mitral valve and annulus was better in flexible ring than in rigid ring. The posterior mitral leaflet was small and rigid after operation. The close of mitral valve depended on anterior leaflet. Conclusions The three-dimensional echocardiography is a useful tool not only in the diagnosis of mitral prolapse but also in evaluating the effect after mitral valve repair.
