透明质酸钠联合地塞米松治疗不可复性关节盘前移位的临床研究

目的 评估透明质酸钠联合地塞米松磷酸钠行关节上腔注射和透明质酸钠单独用药行关节上腔注射对治疗不可复性关节盘前移位的疗效.方法 64例颞下颌关节不可复性盘前移位患者随机分为试验组和对照组,每组32例.试验组采用透明质酸钠与地塞米松磷酸钠联合用药行关节上腔注射,对照组采用透明质酸钠单独用药行关节上腔注射.采用t检验比较两组在张口度和疼痛方面的疗效差异.结果 两组治疗前后在张口度和疼痛方面都有改善,差异有统计学意义.治疗后在疼痛方面两组间差异有统计学意义.结论 透明质酸钠联合地塞米松磷酸钠关节上腔注射和单纯透明质酸钠关节上腔注射对不可复性盘前移位患者改善张口度和缓解疼痛有较好疗效,但在缓解痛疼方面透明质酸钠联合地塞米松磷酸钠优于单纯透明质酸钠.

Sodium hyaluronate acid injection combined with dexamethasone sodium phosphate for treatment of anterior disc displacement without reduction of temporamandibular joint

Objective The purpose of this study was to evaluate the efficacy of sodium hyaluronate acid （HA） injection combined with dexamethasone sodium phosphate （DM） for treatment of anterior disc displacement without reduction （ADDwoR） of temporamandibular joint （TMJ）.Methods Sixty-four patients diagnosed as ADDwoR were divided into study group and control group randomly,32 in each group.A volume of 1 ml of HA and 0.5 ml of DM were injected into the upper cavity in study group while a volume of 1.5 ml of HA only were injected into the upper cavity in control group.Results The maximum mouth opening was increased significantly （P0.01） and jaw pain had also significantly improved （P0.01） after the upper cavity injection in both groups.There was no significant difference between the two groups in increasing mouth opening,but there was significant difference between two groups in reducing pain.Conclusions It seems that it is effective for the improvement of mouth opening of ADDwoR by using the injection of HA injection combined with DM or HA only.These results also suggest that the injection of HA combined with DM can gain more effect in improvement of jaw pain than the injection of HA only.
