回纹型风湿病15例临床分析

报告１５例回纹型风湿病患者。其临床特征为反复发作的四肢关节肿痛，多于数小时或数天内自行缓解。发作间歇期介于２周至１年不等，间歇期内无症状或异常体征。本组患者中受侵的关节依次为掌指关节（８６．３％）、近端指间关节（８０．０％）、腕关节（６６．７％）、膝关节（５３．３％）、肘关节（４０．０％）、踝关节（１３．３％）和跖趾关节（２．０％）。在关节炎发作时，１例伴有低热，３例关节周围出现结节，１例有关节腔

Palindromic rheumatism:a clinical analysis of 15 cases

Palindromic rheumatism (PR) is an uncommon disease. We reported 15 patientswith typical clinical manifestation of this disorder, which was characterized by recurrent episodesof acute arthritis with periarticular inflammation. Each attack of arthritis usually lasted from a fewhours to several days before disappearing completely with no residual articular effect. The affectedjoints included palms, soles, knees and ankles with redness and swelling. Except one patient withlow fever, the other 14 patients were lack of constitutional symptoms and laboratory abnormalitiesduring their episodes of acute arthritis. Among 15 patients, there were 3 patients associated withperiarticular nodule and 1 patient with synovial fluid. At the later stage of the course there were 4patients, whose attacks of arthritis were progressively worsened both in frequency and in intensity, developing the clinical manifestations of rheumatoid arthritis (RA) including the abnormality oferythrocyte sedimentation rate (ESR) in 4 patients, seropositivity of rheumatoid factor in 2 patients, and X-ray bone erosion in one case and MRI abnormality in another case. From the linksbetween the PR and RA we considered that PR may be a specific spectrum of RA.
