椎体压缩骨折术后骨折改善率相关影响因素分析

目的探讨经皮椎体成形术(PVP)对椎体压缩骨折的治疗效果,分析影响椎体压缩骨折术后骨折改善率相关因素。方法对74例椎体压缩骨折患者行PVP治疗。观察患者各项指标,并收集相关资料,采用疼痛视觉模拟评分(VAS)评价疼痛程度。测量患者矢状面Cobb角、骨水泥弥散程度、骨质疏松程度、矢状面椎体压缩率以及术后椎体压缩改善率。结果患者术后3个月的VAS评分、椎体压缩率、Cobb角均较术前显著改善(P<0.05)。术后椎体压缩骨折改善率为34.83%±3.09%。直线相关分析结果显示,病程、术前椎体压缩率、骨水泥弥漫系数与椎体压缩改善率存在一定的相关性(P<0.05)。秩相关分析结果显示,骨质疏松程度与椎体压缩改善率存在一定的相关性(P<0.05)。多元线性回归分析中结果显示,病程、患者骨质疏松程度以及术前椎体压缩率为影响椎体压缩骨折改善率的主要因素(P<0.05)。结论 PVP能够显著改善椎体压缩骨折患者的疼痛、椎体压缩症状及恢复脊柱力学结构的稳定;患者的病程、骨质疏松程度以及术前椎体压缩率与椎体压缩骨折PVP术后改善率密切相关。

The related factors of the improvement rate of vertebral compression fracture

Objective To discuss the healing effect of percutaneous vertebroplasty(PVP) in vertebral compression fracture.Methods 74 vertebral compression fracture patients were treated with PVP in our hospital.Visual analogue scale(VAS) was used to assess patient′s pain degree,and sagittal Cobb angle was measured.Bones cement dispersion degree,osteoporosis degree,patients sagittal vertebral compression ratio and postoperative vertebral compression improvement rate were recorded.Results After 3 months,the VAS,vertebral compression ratio,Cobb angle were improved significantly than before surgery.The results was significant different(P<0.05).Patients′ vertebral compression fracture improvement rate was 34.83%±3.09%.Linear correlation analysis results showed that course of disease,preoperative vertebral compression ratio,bone cement diffuse coefficient and vertebral compression improvement rate had certain relevance(P<0.05).Rank correlation analysis results showed that osteoporosis degree had certain relevance with vertebral compression improvement rate(P<0.05).The linear multivariate regression analysis results showed that the course of disease,osteoporosis degree and preoperative vertebral compression ratio was the main factors influence the vertebral compression fracture improvement rate(P<0.05).Conclusions PVP can significantly improve patients′ pain,vertebral compression symptoms and restore the stability of the spine mechanics.And course of disease,osteoporosis degree,preoperatie vertebral compression ratio is the closely related of vertebral compression fracture PVP surgery′s improvement rate.
