双水平气道正压通气治疗重叠综合征伴呼吸衰竭疗效分析

目的探讨应用双水平气道正压通气对重叠综合征并呼吸衰竭患者的治疗效果。方法选择住院的重叠综合征并呼吸衰竭患者28例,随机分为两组,治疗组在常规治疗基础上(包括氧疗+药物等)给予BiPAP通气,对照组在常规治疗基础上(包括氧疗+药物等)给予CPAP通气。观察比较两组间动脉血气(pH,PaO2、PaCO2)和多导睡眠图仪(PSG)的变化、插管率、病死率、及不良反应。结果治疗后治疗组的pH、PaCO2、AHI、最长呼吸暂停时间、气管插管率、住院死亡率及不良反应率均优于对照组。结论 BiPAP治疗重叠综合征伴呼吸衰竭效果优于CPAP,值得在临床推广。

Analysis of the therapeutic effects of bilevel positive airway pressure ventilation on overlap syndrome complicated with respiratory failure

Objective To investigate the therapeutic effect of bilevel positive airway pressure(BiPAP)ventilation on overlap syndrome complicated with respiratory failure.Methods 28 patients of overlap syndrome complicated with respiratory failure were randomly divided into 2 groups.The patients of the therapeutic group were treated by BiPAP on the basis of routine treatment(oxygen therapy+drugs).and the patients of the control group were treated by CPAP on the basis of routine treatment(oxygen therapy+drugs).The differences of blood gas analysis(pH,PaO2,PaCO2) and Polysomnography,tracheal cannula rate,the mortality rate and adverse reaction rate were observed and compared for the two groups.Results All of pH,PaCO2,AHI,the longest time of apnea,tracheal cannula rate,the mortality Rate and adverse reactions rate of the therapeutic group were superior to those of the control group after treatment.Conclusion BiPAP ventilation has more therapeutic effect in patients with overlap syndrome complicated with respiratory failure than CPAP.and it shoud be expanded in clinical practice.
