TB-SA抗体检测对活动性肺结核临床诊断价值的探讨

目的对TB-SA抗体(Tuberculosis specific Antigen)检测用于活动性肺结核诊断的价值进行评估。方法选取活动性肺结核患者162例(菌阳肺结核78例,菌阴肺结核84例),无结核病患的健康查体者115人,同时检测血清TB-SA抗体、TB-DOT抗体和TB-PCR,入选的活动性肺结核患者连续三天留取3份晨痰标本,进行抗酸杆菌检查和分支杆菌培养。结果活动性肺结核菌阳患者检测血清TB-SA抗体敏感性为85.9%,菌阴者为72.6%,健康人群特异性为86.1%。结论 TB-SA抗体检测是一种快速、简便、有较高敏感性和特异性的辅助诊断活动性肺结核的免疫学方法。

Clinical value of TB-SA antibody in diagnosis of active pulmonary tuberculosis

Objective To evaluate the value of TB-SA(Tuberculosis-specific Antigen) test in the diagnosis of active pulmonary tuberculosis.Methods 162 active pulmonary tuberculosis patients(78 bacteriological-positive Pulmonary tuberculosis Patients,84 bacteriological-negative pulmonary tuberculosis patients) and 115 healthy people with no tuberculosis sera specimens from the 277 cases were studied by TB-SA antibody,TB-DOT antibody and TB-PCR detection.The selected patients with active pulmonary tuberculosis took three morning sputum on three consecutive days which were submitted for acid-fast bacillus and Mycobacterium tuberculosis smear culture.Results The sensitivity of TB-SA test with bacteriological-positive active pulmonary tuberculosis patients was 85.9% and bacteriological negative pulmonary tuberculosis patients was 72.6%.The specificity of TB-SA antibody test with healthy pepole was 86.1%.Conclusions The TB-SA antibody test was rapid,simple,relatively sensitive and specific immunological method for auxiliary diagnosis of active pulmonary tuberculosis.
