椎体成形术治疗椎体压缩骨折2年随访分析

目的探讨经皮椎体成形术(PVP)和经皮椎体后凸成形术(PKP)治疗骨质疏松性椎体压缩骨折(OVCF)的短期及近期临床疗效。方法 41例OVCF患者中行PVP治疗11例,PKP治疗16例,非手术治疗14例。对患者的疼痛程度采用视觉模拟评分法(VAS)评分,分别记录患者入院时及手术或非手术治疗后2 d的VAS评分,分析PVP、PKP的短期临床疗效。并对41例OVCF患者进行2年的随访,分别记录手术或非手术治疗后2年的VAS评分,分析3种方法治疗OVCF的近期临床疗效。结果手术治疗2 d后VAS评分明显下降。随访2年后PVP组及PKP组VAS评分与非手术治疗组比较差异无统计学意义(P>0.05),PVP组VAS评分较PKP组低,差异有统计学意义(P<0.05)。所有手术患者均未发现重大并发症。结论 PVP、PKP均能迅速缓解患者疼痛,短期临床疗效良好,与非手术治疗相比近期临床疗效无差异;与PKP相比PVP近期疗效较好。

Clinic efficacy after percutaneous vertebroplasty or percutaneous kyphoplasty:A two-year follow-up

Objective To explore the short-term and preliminary term efficacy of percutaneous vertebroplasty(PVP) and percutaneous kyphoplasty(PKP) for osteoporotic vertebral compressive factures(OVCF).Methods Retrospective analysis was done about the data of 41 OVCF patients.PVP was performed at 11 OVCF patients.PKP was performed at another 16 OVCF patients,and the other 14 OVCF patients received nonoperation therapy.The change of visual analogue score(VAS) were measured on admission and 2 days after treatment.The VAS score about all the patients were also recorded 2 years after treatment to determine short-term effect.Results The VAS score of the OVCF patients declined significantly 2 days after either PVP or PKP.While there was no significant difference between operation groups with nonoperation therapy group in VAS score after 2 years(P>0.05).The VAS score of PVP group was lower than PKP group after 2 years(P<0.05).Conclusions Immediate pain relief is definite after PVP or PKP.After 2 years,the clinical outcome after PVP or PKP is similar with nonoperation therapy,and the effect of PVP is better than PKP.
