种植体初期稳定性与骨密度关系的研究

摘    要:目的:通过螺旋CT检查不同区域颌骨骨密度并与种植体初期稳定性相比较,观察二者之间的相关性。方法:20例接受种植修复患者,术前行颌骨CT扫描检测缺牙区骨密度,术中记录种植体植入扭矩并通过动度测量仪测量植入后种植体动度,将测量结果进行统计学比较。结果:20例患者共植入39颗种植体,按照解剖区域分为下颌前牙区、下颌后牙区、上颌前牙区、上颌后牙区,各部位的平均骨密度为:776.66 Hu、746.72 Hu、431.68 Hu、193.28 Hu;平均植入扭力为:33.56 Ncm、30.80 Ncm、27.66 Ncm、25.50 Ncm;平均动度为:75.61、70.85、67.96、58.59。统计学分析发现不同缺牙区域中牙槽骨骨密度、种植术后扭力以及种植体动度测量值比较均具有统计学差异(P〈0.05);Kruskal Wallis秩和检验三组不同数据具有显著相关性。结论:通过术前CT扫描检测骨密度可以有效地预测术后种植体初期稳定性,从而有可能提高种植成功率,减少失败率。

Abstract:Objective:to observe the relationship between primary stability of implants and bone density measured by insertion torque(Ncm) and resonance frequency analysis.Method:20 patients underwent 39 implants.In anterior mandible,posterior mandible,anterior maxilla and posterior maxilla region,the average bone density values were 776.66 Hu、746.72 Hu、431.68 Hu、193.28 Hu;the average insertion torque were 33.56 Ncm、30.80 Ncm、27.66 Ncm、25.50 Ncm;the average resonance frequency were 75.61、70.85、67.96、58.59.Statistic analysis showed that in different region the bone density,insertion torque,resonance frequency have statistics difference(P 0.05).Kruskal Wallis analysis showed the data have significant correlation in three group.Conclusion:Through the preoperative CT scanning for detecting bone mineral density can be effective ways to predict primary stability of implant,which may improve the success rate,reduce the failure rate.
