2型糖尿病患者心率变异性与自主神经病变研究

目的研究2型糖尿病(T2DM)患者心率变异性(HRV)与自主神经病变关系。方法50例2型糖尿病患者按病程长短分成2组:病程>10年组(Ⅰ组)27例,病程<10年组(Ⅱ组)23例;健康对照组(Ⅲ组)25例。测定每例HRV时域指标,3组对比分析。结果3组HRV降低发生率分别为62.96%、8.70%、0.00%,差异具有显著性;3组SDNN、SDANN Index、SDNN Index各组间比较差异均有显著性;rMSSD、PNN50 I组与III组比较差异具有显著性,II组与III组比较差异无统计学意义。结论HRV是早期判定T2DM患者自主神经病变指标,T2DM患者交感神经张力明显增高,随病程延长,迷走神经功能有不同程度损害。

Study on the heart rate variability and autonomic neuropathy in patients with type 2 diabetes

Objective To study the relationship between the heart rate variability(HRV) and autonomic neuropathy in patients with type 2 diabetes.Methods Fifty cases of type 2 diabetes were divided into two groups:the group in which the course of diease was more than 10 years(group I) was 27 cases,the other group in which the course of diease was less than 10 years(group II) was 23 cases.Healthy control group(group III) was 25 cases.HRV indices of each case were detected,and were analyzed in the three groups.Results The HRV depress incidences in the three groups were 62.96%,8.70% and 0.00% respectively.The distinction was significant.The distinctions of SDNN,SDANN index and SDNN index in the three groups were significant respectively.The distinction of rMSSD,PNN50 in group I and III was significant.The distinction of rMSSD,PNN50 in group II and III was not significant.Conclusion HRV is the earlier index to assess the autonomic neuropathy in T2DM.The tension of adrenergic nerve in T2DM manifests increases,as the course of the disease lasting,the vagus nerve function is damaged in different degrees.
