B超引导下肺脓肿经皮穿刺抽吸术

目的　探讨 Ｂ 超作为导向手段，经皮穿刺抽吸诊断、治疗周围型肺脓肿的方法及临床价值。方法　对２６ 例周围型肺脓肿病人行 Ｂ 超引导下经皮穿刺抽吸及注药治疗。抽吸物送检，做细菌培养及药敏试验。穿刺过程中对脓腔进行反复灌洗，冲洗完毕，脓腔内常规注入抗生素进行介入治疗。结果　２６ 例病人均具备典型肺脓肿超声表现，２４ 例一次穿刺成功抽得脓液送检。其中８ 例病人根据药敏报告，调整治疗方案后，取得满意疗效。４ 例病人出现少量气胸。肺脓肿治愈率为９２ ．３ ％ 。病人平均住院２１ｄ 。结论　 Ｂ 超引导下肺脓肿经皮穿刺抽吸术，具有定位准确、创伤小、安全、取得标本可靠等优点，能明显提高肺脓肿的治愈率，缩短疗程，有较高的临床实用价值。

B-ULTRASOUND GUIDED PERCUTANEOUS ASPIRATION IN THE TREATMENT OF LUNG ABSCESS

Objective To explore the applied approaches and to evaluate the clinical value of B ultrasound percutaneous aspiration in the treatment of lung abscess.Methods B-ultrasound guided percutaneous aspiration was conducted in 26 cases of periferal lung abscess.Aspirates were cultured and the sensitivity test of antibiotics was done.The lung abscess cavity was then irrigated and antibiotics were injected and kept in the cavity for treatment.Results Typical ultrasound signs of lung abscess were detected in the 26 cases.Successful aspirations were got in 24 cases(92.3%).Eight patients had their treatment plans adjusted according to the sensitivity pattern of antibiotics and got satisfactory effects.Mild pneumothorax was found in 4 cases(15.4%).The curative rate of the lung abscesses was 92.3% .The average time of hospitalization was 21 days.Conclusion B ultrasound guided percutaneous aspiration has been proved to be a reliable and light traumatic method in the treatment of lung abscess.
