高频超声对浅表性血管瘤诊断的评价

摘    要:目的 以高频超声观察浅表性血管瘤的内部结构、形态分型和声像图特征,用彩色多普勒检测其血供状况,以期对临床选择治疗方式提供依据。方法 对53例浅表性血管瘤采用探头加压试验观察瘤体受压缩后形态变化及血流情况,频谱多普勒检测血流性质和参数信息。结果 53例血管瘤中声像图特征分为类实性15例,占28%,表现为内部回声强弱不等,压缩变化小、血供少;混合型26例,占50%,表现为网格状的实性组织与液性暗区相间,压缩变化显,血流信号较丰富;以液性暗区为主型12例,占22%,表现为大小不等或相互交通的管状液性暗区,界限模糊,压缩性好,显示动静脉血流流速曲线。结论 高频彩色多普勒超声显示浅表性血管瘤的形态结构清晰,诊断可靠,可提供临床选择治疗方案的依据。

Abstract:Objective To observe inner texture, morphologic types and sonographic characters of superficial hemangioma by using high-freuency ultrasound, and to detect blood supply by color Doppler, so as to contribute clinical management. Methods We observed 53 patients with superficial hemangioma, in terms of morphologic changes, color flow signals and flow parameters on color Doppler imaging after compressing test with probe. Results The sonogaphic characters of the hemangiomas of 53 patients were classified as three types. Solid type (15,28%), characterized by homogeneous, a little morphologic change under compressing, and scarce color signal. Mixed type (26,50%), characterized by the mixture of netty solid tissue and liquid-echo areas, notable morphologic changes under compressing, and sufficient blood supply. Liquid-echo dominating type (12,22%), characterized by tubular liquid-echo areas with diffrent size or interconnected each other, obscure border, good compressibility with arteriovenous waveform. Conclusion High-frequency color Doppler ultrasonography contributes to the clear classifications of morphologic sturcture of superficial hemangioma and reliable diagnosis, and is an ideal tool for selective clinical management.
