粒细胞肉瘤18例临床分析

目的探讨粒细胞肉瘤患者的临床特点、治疗方法和预后。方法回顾性分析18例粒细胞肉瘤患者临床特点、治疗方法,并进行随访,应用Kaplan-Meier法行生存分析和Log-Rank检验行差异显著性检验。结果 18例粒细胞肉瘤患者中男10例,女8例,中位年龄40岁。好发部位为椎管、淋巴结、乳腺、骨、鼻腔、眼眶等。18例粒细胞肉瘤3年总生存率12.1%,化疗与不化疗患者的3年生存率比较差异有统计学意义(P=0.001),单独化疗与放化疗结合的3年生存期比较差别无统计学意义(P=0.263),合并CML和MDS-RAEB的患者3年生存期与其他患者无明显差异(P0.05)。结论粒细胞肉瘤应早期诊断,及时化疗,局部放疗不能提高生存率。

Granulocytic Sarcoma:A Clinical Analysis of 18 Cases

Objective To investigate the clinical manifestations, treatment and prognosis of granulocytic sarcoma. Methods A retrospective analysis was made of 18 cases of granulocytic sarcoma including the clinical characteristics and treatment. The statistic difference was tested by using Kaplan-Meier survival analysis and line of Log-Rank test. Results of all the 18 cases,10 were males and 8 females. The median age was 40. The predilection sites included canal, lymph nodes, breast, bone, nasal cavity, orbit etc. The total three-year survival rate was 12.1%, with a significant difference between the three-year survival rate of patients with chemotherapy or not（P=0.001 ）. No significant difference was showed between the three years survival of patients with chemotherapy alone or chemotherapy combined with radiotherapy（P=0.263）.And the three-year surriral of patients combined CML or MDS-RAEB was about the same with guanulocutic sanoma single （P〉0.05）. Conclusion The early diagnosis and chemotherapy of granulocytic sarcoma should be in time, and local radiotherapy can not improve the survival rate.
