肝动脉系统三维造影剂增强磁共振血管成像

摘    要:目的 评价三维造影剂增强磁共振血管成像(3D CE MRA)显示肝动脉系统的价值。方法 40例肝细胞癌患者行肝动脉系统3D CE MRA成像。评价3D CE MRA显示肝动脉系统的显影质量,诊断肝动脉系统解剖和变异的能力。3DCEMRA的结果与常规血管造影术(32例)或手术(8例)对照。结果所有病例,3D CE MRA清楚显示肝动脉系统。3D CE MRA准确显示所有31例肝动脉系统的正常解剖。3D CE MRA检出9例解剖变异中的7例。3D CE MRA显示肝动脉解剖与变异的总体准确性为95%。3D CE MRA又可显示肝内肿瘤染色、动静脉瘘形成以及肝动脉受累情况。结论 3D CE MRA是一项显示肝动脉系统的有用技术。

Abstract:Purpose To evaluate the usefulness of three-dimensional contrast-enhanced MR angiography (3D CE MRA) in the evaluation of hepatic arterial system. Methods 3D CE MRA of hepatic arterial system was performed in 40 patients with hepatocellular carcinoma.3D CE MRA was evaluated in terms of image quality,depiction of hepatic arterial anatomy and variation.Results of 3D CE MRA were correlated with findings on conventional angiography in 32 patients or at surgery in 8 patients. Results The hepatic arterial system was well visualized in all cases.3D CE MRA correctly demonstrated all 31 patients with normal hepatic arterial anatomy.Arterial anomalies were detected by 3D CE MRA in 7 of 9 patients.The overall accuracy of 3D CE MRA in depiction of the arterial anatomy and variation was 95%.3D CE MRA also showed intrahepatic tumor stain,arteriovenous fistula and involvement of hepatic artery by tumors. Conclusions 3D CE MRA is a useful technique for the evaluation of the hepatic artery.
