腔外型胃间质瘤的MSCT诊断的临床分析

目的:探讨腔外型胃间质瘤(gastric stromal tumor,GST)的MSCT表现。方法:分析9例经手术病理与免疫组化证实腔外型GST的临床资料和MSCT表现。结果:9例腔外型GST位于腹腔内胃周间隙。边界模糊不清7例,部分清楚部分不清楚2例。单囊型4例,多囊型5例。肿瘤大小平均165mm×126mm.。囊壁厚薄不均,囊内见少量条片状软组织密度影。增强扫描动脉期囊壁、囊内分隔及囊内软组织成份明显强化,低密度区未见强化。实质期强化幅度增加。5例病灶边缘部分见多发成簇状或线状排列细小血管。MIP像上3例腔外型GST可见肿瘤供血动脉。结论:腹腔内胃周间隙的囊性肿块为腔外型间质瘤的主要表现形式,仔细分析肿瘤的各种影像表现结合合理的后处理重建技术,特别是肿瘤供血动脉的显示,能明显提高腔外型GST定位定性正确率。

MSCT diagnosis of extraluminal gastric stromal tumors

Objective：To investigate the MSCT performance of extraluminal gastric stromal tumors（gastric stromal tumor,GST）.Methods：To analyse the clinical data and MSCT performance of extraluminal GST confirmed by surgery pathology and immunohistochemistry in 9 cases.Results：9 cases of extraluminal GST located at the gap around stomach in the adomina.7 cases of blurred boundaries,some is clear and some is not clear in 2 cases.Unicystic 4 cases,multiple extraluminal in 5 cases.The average tumor size 165 mm×126 mm.Cyst wall thickness is uneven.a little banding and sheet soft parenchyma density shadow was seen in the systica.The systica wall,compartmentation and parenchyma element in the systica were obviously enhanced and the low density area was not enhanced by enhancement scan during arterial phase.The enhanced rate was increased during parenchymal phase.there were crinose minute blood vessels arranged tuftedly and wirelikely at the borders of focuses of five cases.Tumor-feeding arteries were seen on MIP imaging in 3 cases of extraluminal GST.Conclusion：The mass at the gap around stomach in the abdomin is the chief pattern of manifestation of extraluminal GST,a careful analysis of the tumor with a reasonable performance in a variety of image reconstruction of post-processing technology,especially the display of the tumor feeding arteries,can improve the extraluminal GST positioning qualitative accuracy.
