CRRT治疗重症横纹肌溶解综合征合并急性肾功能衰竭

目的观察CRRT治疗重症横纹肌溶解综合征合并急性肾功能衰竭的临床疗效。方法对2004～2007年抢救的36例重症横纹肌溶解综合征合并急性肾功能衰竭患者进行CRRT治疗的临床资料进行回顾性分析,观察肌红蛋白、肌酸磷酸激酶、乳酸脱氢酶、肌酐、尿素氮、电解质以及生命体征变化。结果29例患者经CRRT治疗后,肾功能恢复正常,痊愈出院。7例患者因病情延误死于多器官功能衰竭(MODS)。结论对于重症横纹肌溶解综合征合并急性肾功能衰竭应尽早进行CRRT治疗,从而改善患者的预后。

CRRT in the treatment of severe rhabdomyolysis complicated with acute renal failure

Objective To investigate the clinical effects of CRRT and its influence on survival of patients with severe rhabdomyolysis complicated with acute rena1 failure (ARF). Method The clinical data of 36 patients with severe rhabdomyolysis complicated with ARF undergone CRRT (from 2004 to 2007 yr.) were reviewed and analyzed retropectively, which the changes of Mb, CPK, LDH, K+, Ca2+, CO2 -CP, ALT, AST, AKP, BUN, Scr, living signs and so on in the treatment were observed. Result Among the 36 cases 29 were recovery after CRRT, 7 died from MODS due to delayed treatment. There were no complications such as continuous hypotension during the therapy. Conclusion CRRT for treating patients with severe ARF might be helpful to stabilize hemodynamics, eliminate the solute, in favor of nutrition abeted, and eliminate inflammation response, thus improving the prognosis of patients with ARF. Otherwise, correct nursing care may be important.
