| 摘 要： | 目的:探索首诊医师负责制在慢性病患者健康管理中的效果,为全国其它城市及地区的社区慢性病健康管理提供参考。方法:将疾病诊疗中的"首诊医师负责制"原理引入到慢性病患者健康管理中的各个环节。通过问卷调查了解慢性病患者健康管理首诊负责制的实施效果。结果:首诊医师负责制实施后首诊测血压比例达到94.52%,筛查出来的慢性病病人数同比实施前增长228.76%,纳入管理人数同比增长169.23%,管理效果差异也具有统计学意义。结论:首诊医师负责制是一种有效的慢性病患者健康管理方式,能够促进社区慢性病健康管理工作的落实。
