腹膜后腹腔镜与开放性分支肾动脉阻断肾部分切除术治疗肾肿瘤的效果比较

目的：比较腹膜后腹腔镜与开放性分支肾动脉阻断肾部分切除术治疗肾肿瘤的临床效果。方法取2010年2月~2012年3月本院泌尿外科收治的34例肾肿瘤患者，随机将其分为对照组与实验组各17例，对照组接受开放性分支肾动脉阻断肾部分切除术，实验组在腹腔镜支持下实施分支肾动脉阻断肾部分切除术。观察两组术中出血量、手术时间、术后住院时间、热缺血时间、并发症发生率以及胃肠功能恢复情况。结果与对照组比较，实验组手术时间更长，术中出血量更少，术后住院时间、热缺血时间及胃肠功能恢复时间更短，并发症发生率更低，组间差异有统计学意义（P〈0.05）。结论相比于开放性手术，虽然腹腔镜手术用于肾肿瘤的手术时间长，但出血量少，效果确切，安全性较高，术后并发症少，值得临床推广应用。

Comparative efficacy of retroperitoneal laparoscopic and open renal artery branches blocking partial nephrectomy in treatment of kidney cancer

Objective To compare the effects of retroperitoneal laparoscopic and open renal artery branches blocking partial nephrectomy in treatment of kidney cancer. Methods 34 cases of patients with kidney cancer in our hospital from February 2010 to March 2012 were selected and randomly divided into control group and experimental group,17 cases in each group.The control group were treated with open branch renal artery occlusion partial nephrectomy,the ex-perimental group were treated with laparoscopic renal artery occlusion partial nephrectomy.Blood loss,operative time, postoperative hospital stay,warm ischemia time,complication rates and recovery of gastrointestinal function in two groups were observed. Results Compared with the control group,the experimental group had longer operative time,less blood loss,shorter postoperative hospital stay and warm ischemia time and gastrointestinal fuction recovery time,the complica-tion rate was lower,there was statistical difference （P〈0.05）. Conclusion Compared with open surgery,although laparo-scopic surgery for renal tumors time is longer,but has less bleeding,exact effect,higher security,fewer postoperative com-plications,and thus is worthy of clinical application.
