螺旋CT对肺动脉栓塞的诊断价值

目的评价螺旋CT对肺动脉栓塞的诊断价值。方法对 2 0例行螺旋CT检查的肺动脉栓塞患者进行回顾性分析。使用SOUATOMEmotion螺旋CT机 ,连续容积增强扫描。结果共分析 2 2 0支肺动脉分支 ,受累 12 6支 (5 7.3% )。造影增强后直接征象 :部分性充盈缺损 76支 ,环形附壁充盈缺损 2 7支 ,完全性充盈缺损 15支 ,中心性充盈缺损 (轨道征 ) 8支 :间接征象 :肺纹理稀 5例 ,肺梗塞 3例 ,肺动脉主干增宽 10例 ,胸腔积液 2例。结论在肺动脉栓塞诊断诊断中 ,螺旋CT是一种安全可靠 ,快速无创的方法 ,对肺动脉塞的临床诊断和疗效评价均有重要价值。

The application of spiral CT for the Diagnosis of Pulmonary Embolism

Objective To study the diagnosis evaluation of spiral CT for Pulmonary embolism (PE).Methods 20 patients had PE demonstrated by spiral CT,Continuous volume enhanced scan was used with SOMATOM Emotion scanner.Results Among 220 pulmonary artery branches in 20 cases, 126 branches were involved with PE (57.3%). The direct signs of CT are: Partial filling defect (76 branches), mural filling defect (27 branches), total filling defect (15 branches), and railway-track sign (8 branches). Local pulmonary oligemia, pulmonary infarction, enlargement of pulmonary artery trunk and pleural effusion are the indirect signs. Conclusion Spiral CT is a noninvasive and effect method for diagnosing PE, It could play an important role in clinical diagnosis and evaluating curative effect of PE.
