白三烯拮抗剂辅助治疗儿童呼吸道支原体感染疗效观察

摘    要:目的肺炎支原体是儿童呼吸道感染的主要病原,发病率高,除引起呼吸道粘膜损伤,尚可作为变应原,引起气道的高反应,导致长程咳嗽,甚至诱发哮喘的发生发展1]。白三烯受体拮抗剂(孟鲁司特)可阻断肺炎支原体感染后作为变应原刺激呼吸道的炎症反应,降低气道的高反应性。方法选择2006年3月-2008年2月在我院呼吸专科就诊收治入院的760例肺炎支原体感染患儿,随机分为观察组和对照组,两组均采用常规抗支原体的基础治疗,观察组同时予以孟鲁司特治疗4d和7d后观察咳嗽情况。结果肺炎支原体感染后常规应用阿奇霉素抗支原体及对症治疗,辅以孟鲁司特进行治疗后,咳嗽缓解与明显有效明显高于对照组。结论选择白三烯受体拮抗剂,可阻断肺炎支原体感染后作为变应原刺激呼吸道的炎症反应,降低气道的高反应性,减少粘液分泌、缓解支气管痉挛,可以缩短咳嗽病程。

Abstract:Objective Mycoplasma pneumoniae is the main pathogen in upper respiratory tract infection among children,and the morbidity is also high.In addition to causing respiratory mucosa,it can act as an allergen to cause airway hyperresponsiveness,result in long-range cough,even trigger or develop asthma1].Leukotriene receptor antagonist montelukast can block mycoplasma pneumoniae that stimulates the inflammatory response of the respiratory tract as an allergen after infection,and lower airway hyperresponsiveness.Methods Totally 760 children infected with mycoplasma pneumoniae from March 2006 to February 2008 in respiratory disease department of our hospital were selected to randomly assign into the observation group and the control group.Both groups took conventional anti-mycoplasma therapy,but the observation group simultaneously took montelukast for 4 days and 7 days,then their cough conditions were observed and compared.Results Mycoplasma pneumoniae infection after application of conventional anti-mycoplasma therapy(azithromycin) and symptomatic treatment,supplemented by montelukast special treatment,the symptom remission of cough in the observation group was significantly better than that of the control group.Conclusions Leukotriene receptor antagonist can block mycoplasma pneumoniae that stimulates the inflammatory response of the respiratory tract as an allergen after infection,lower airway hyperresponsiveness,reduce mucus secretion,ease bronchial spasm,and shorten the duration of cough.
