食管鳞癌两种食管重建方式术后并发症的比较

[目的]探讨管状胃食管重建在食管鳞癌手术的安全性、可行性及术后并发症的处理措施.[方法]回顾分析2007年9月至2012年9月收治的72例食管鳞癌患者的临床资料.其中42例行管状胃食管重建,30例行全胃食管重建,对比分析两种手术患者术后并发症,包括食管吻合口瘘、肺部感染、腹部切口感染、返流性食管炎、吻合口狭窄.[结果]管状胃食管重建术后各种并发症发生率均低于全胃食管重建术,差异有统计学意义（P＜0.05）.[结论]在食管鳞癌手术中,与全胃食管重建相比,管状胃食管重建在减少术后并发症方面具有明显优势,手术安全可靠,适于基层医院开展.

Comparison of Complications on Two Esophageal Reconstructions in Patienls with Esophageal Squamous Carcinoma

[Objective]To explore the safety and feasibility of tubular gastroesophageal reconstruction in the treatment of esophageal squamous cell carcinoma and the management measures of postoperative complications.[Methods] Clinical data of 72 patients with esophageal squamous cell carcinoma from Sept.2007 to Sept.2012 were analyzed retrospectively.Among all patients,42 patients underwent tubular gastroesophageal reconstruction and 30 patients underwent full gastroesophageal reconstruction.Postoperative complications including esophageal anastomotic fistula,pulmonary infection,abdominal wound infection,reflux esophagitis and anastomotic stricture were compared between two groups.[Results] The incidence of postoperative complications in tubular gastroesophageal reconstruction group was lower than that in full gastroesophageal reconstruction group,and there was significant difference（P ＜0.05）.[Conclusion] Compared with full gastroesophageal reconstruction,tubular gastroesophageal reconstruction for the treatment of esophageal squamous cell carcinoma has obvious advantages in reducing postoperative complications,and is safe,reliable and suitable for primary hospital.
