心型脂肪酸结合蛋白在不稳定型心绞痛危险分层及分级中的表达及意义

[目的]探讨心型脂肪酸结合蛋白（H-FABP）在不稳定型心绞痛（UAP）危险分层及分级中的表达.[方法]150例UAP患者作为试验组,其中低危组54例、中危组60例、高危组36例;按传统Proudilit狭窄分级法将冠脉狭窄分为Ⅰ～Ⅵ级.另选30例体检正常者作为对照组,检测并比较各组血清H-FABP变化.[结果]试验组血清H-FABP明显高于对照组（P＜0.01）;UAP患者低危组、中危组、高危组比较均有显著统计学差异（P＜0.01）;血管狭窄程度愈高,H-FABP表达水平愈高,其中Ⅳ级病变组较Ⅲ级病变组,Ⅵ级病变组较Ⅳ级病变组H-FABP表达水平显著上升（P＜0.01）,Ⅳ级较Ⅴ级病变组稍上升,但差异无显著性（P＞0.05）.[结论]血清H-FABP水平随UAP危险程度增加而升高,根据其水平可判断UAP患者冠脉病变的严重程度.

Expression and Significance of Heart-type Fatty Acid-binding Protein in Risk Stratification and Classification of Unstable Angina Pectoris

[Objective]To explore the expression of heart-type fatty acid-binding protein（H-FABP） in risk stratification and classification of unstable angina pectoris（UAP）.[Methods] Totally 150 UAP patients were taken as experiment groups including low-risk group（n =54）,middle-risk group（n =60） and high-risk group （n =36）.According to Proudilit stenosis classification,coronary artery stenosis was divided into grade Ⅰ ～Ⅵ.Other 30 healthy people were selected as control group.Serum H-FABP was detected and compared among groups.[Results] Serum H-FABP in UAP group was obviously higher than that in control group（P ＜ 0.001）,and there was significant difference among low-risk group,middle-risk group and high-risk group（P ＜0.001）.The more serious the artery stenosis was,the higher the H-FABP expression was.The expression of H-FABP in grade Ⅳ lesion group was markedly higher than that in grade Ⅲ lesion group,and that in grade Ⅵ lesion group was higher that in grade Ⅳ lesion group（P ＜0.01）.Compared with grade V lesion group,the expression of H-FABP in grade Ⅳ lesion group was slightly increased,but there was no significant difference（P ＞0.05）.[Conclusion]Serum H-FABP is increased with the elevating of risk level of UAP.Serum HFABP can predicate the severity of coronary lesion in UAP patients.
