摘    要:目的探讨血清游离脂肪酸和2型糖尿病合并心血管自主神经病变(DCAN)的相关性。方法选取屯昌县人民医院收治的2型糖尿病合并心血管自主神经病变的患者为观察组,未合并的患者为对照组,再选取健康志愿者作为空白对照组。研究对象均测定甘油三酯(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、游离脂肪酸(FFA)以及心血管自主神经功能。将糖尿病患者归为一个整体,以合并自主神经病变为因变量Y(有=1,无=0),以TC、TG、LDL-C、HDL-C和FFA为自变量进行Logistic多元回归分析。结果观察组患者各项指标检查结果和心血管自主神经各项检查结果与对照组相比,差异均有统计学意义(P0.05),观察组与对照组的自变量TC、TG、LDL-C、HDL-C和FFA经Logistic多元回归分析,仅FFA比较差异有统计学意义(P0.05)。结论血清FFA升高是2型糖尿病患者合并心血管自主神经病变的危险因素。

Abstract:Objective To investigate correlation between the serum free fatty acids (FFA)and diabetic cardiovascular autonomic neuropathy(DCAN)in patients with type 2 diabetes. Methods The 80 patients with type 2 diabetes in Tunchang County People' s Hospital were divided into two group: observation group with DCAN and control group without DCAN. 40 health controls were selected in blank control group. Triglyceride (TG), total cholesterol (TC), high density lipoprotein cholesterol (HDL-C), low density lipoprotein cholesterol (LDL-C), free fatty acid (FFA), and cardiovascular autonomic function were measured. The muhi-logistic regression analysis were applied with autonomic neuropathy as the dependent variable Y ( a = 1, no = 0 ) and with TC, TG, LDL-C, HDL-C, and FFA as independent variables. Results The difference of indicators of DCAN between observation group and control groups was statistically significant (P〈0.05); FFA were statiscally significant in type 2 diabetes complicated with cardiovascular autonomic neuropathy by multi-logistic regression analysis (P〈0.05). Conclusion Increased concentration of FFA was the risk factor for type 2 diabetic patients with cardiovascular autonomic neuropathy.
