恶性淋巴瘤血清CA125的表达及临床意义

目的研究恶性淋巴瘤(ML)中血清癌抗原125(CA125)水平变化,并探讨其临床意义。方法收集广州市南沙中心医院和广州市第一人民医院血液内科确诊116例ML患者,以同期入院的慢性再生障碍性贫血患者为对照,检测ML血清CA125水平,分析血清CA125水平与临床特征、分期、β2-MG、LDH、治疗反应、组织类型及预后等关系。结果116例ML患者,血清CA125水平升高有54例,占46.6%,ML初治组患者血清CA125水平较对照组、治疗后部分缓解(Partial remission,PR)或完全缓解组(Complete remission,CR)升高,差异有统计学意义(P0.05),CR组较对照组差异无统计学意义,而复发或进展组(Progressive disease,PD)血清CA125水平较CR组再次升高。ML初治时血清CA125水平与β2-MG、LDH、分期、渗出、B症状、结外病变数目、IPI有明显相关性,血清CA125水平正常组较升高组CR率明显升高(P0.05)。结论血清CA125水平可反映ML的侵袭性、治疗反应,对ML的分期、预后具有重要临床价值;血清CA125水平可能在HL中有一定临床价值。

Expression of serum CA125 in patients with malignant lymphoma and its clinical significance

Objective To study the level of serum CA125 in patients with malignant lymphoma and its clinicalsignificance. Methods Total 116 cases with ML were selected and the inpatients with chronic aplastic anemia were chosenas control group at the same time, then the level of serum CA125 of ML patients at different stage was analyzed to explore therelationship of serum CA125 level with clinical features, staging, level of 132-MG and LDH, treatment response, histologicalsubtypes and prognosis. Results The levels of serum CA125 in 54 patients with ML increased, accounting for 46.6% （54/116）.Average serum CA125 levels in ML patients after initial treatment was significantly higher than those in the controls, in partialremission and in complete remission patients. There was no significant difference of CA125 levels between the completeremission patients and controls. The CA125 levels in patients with recurrent or progressive diseases elevated again. It wasrevealed that CA125 levels had significant positive correlations with β2-MG, LDH level, stage, exudation, B-symptoms, extranodal involvement and high IPI . Patients with normal CA125 levels showed a significantly elevation of complete remission（CR） rate, compared with the group with high levels（P〈0.05）. Conclusion Serum CA125 level can reflect the invasion of MLand is of important clinical value in staging, monitoring response to treatment and estimating prognosis in patients with ML.Serum CA125 level may have some clinical value in staging and monitoring response to treatment in patients with HL.
