经皮腔内冠状动脉成形术后心率变异性变化及心肌梗塞部位的相关性分析

对６５例不同部位陈旧性心肌梗塞（ＯＭＩ）患者采用频域和时域法分析经皮腔内冠状动脉成形术（ＰＴＣＡ）前、后的心率变异性（ＨＲＶ），旨在了解不同部位ＯＭＩ患者ＨＲＶ的变化及ＰＴＣＡ术对患者自主神经系统的影响。结果：不同部位ＯＭＩ患者的ＨＲＶ无明显差异，而ＰＴＣＡ术后ＨＲＶ指标中的高频段（ＨＦ）、标准差均较术前增加，低频段（ＬＦ）及ＬＦ／ＨＦ较术前降低。说明ＰＴＣＡ术后交感神经活性降低，迷走神经活性增强。提示ＰＴＣＡ术后可能改善冠心病患者的远期预后。

The changes of heart rate variability after PTCA and the study of relationship with the site of myocardial infarction

Heart rate variability(HRV) was analyzed in 65 patients with old myocardial infarction before and after percutaneous transluminal coronary angioplasty(PTCA),in order to observe whether there was a relation between HRV and the site of myocardial infarction and to observe the effect of PTCA on autonomic nervous system,The results showed that there was no significant difference between HRV and the infarct site before and after PTCA,but HF,SDNN were significantly increased and LF,LF/HF were significantly reduced at six months after PTCA compared with that before PTCA (P<0.05 in all).It suggested that the sympathetic activity was diminished and the vagal activity was raised after PTCA.It was possible that successful PTCA could improve the longterm prognosis of patients.
