急慢性肝炎和肝硬化患者上消化道黏膜病变与幽门螺杆菌感染的临床研究

目的观察肝病患者上消化道黏膜病变及HP感染的状况,探讨其相关性。方法对577例急慢性肝炎(急性肝炎74例,慢性肝炎232例)和肝硬化(271例)患者进行胃镜检查及HP检测。结果所有患者均存在各自不同的上消化道黏膜病变:发现食管炎37例(6.41%);食管、胃底静脉曲张271例(46.97%);慢性胃炎560例(97.05%);胃溃疡46例(7.97%);门脉高压性胃病52例(9.01%);胃癌2例(0.35%);十二指肠溃疡51例(8.84%);十二指肠炎34例(5.89%)。HP检出率:急性肝炎32.43%(24/74),慢性肝炎43.97%(102/232),肝硬化38.75%(105/271)。结论在肝病患者中普遍存在上消化道黏膜病变,但HP感染率不高于文献报道的非肝病患者,说明肝病患者不是HP感染的高发人群,其上消化道黏膜病变的发生与肝病本身的致病因素有关。

The clinical research on the pathological change of gastric mucosa and helicobacter pylori infection rate in patients with liver diseases

Objective To research the gastric mucosa pathological changes and helicobacter pylori (HP) infection in patients with liver diseases. Methods 577 cases with liver disease including acute hepatitis ( 74),chronic hepatitis (232) and cirrhosis (271) were checked by gastro scope and HP test.Result According to gastro scope examination,all the patients had pathological changes in upper alimentary cabal,37 (6.41%)cases were esophagitis; 271 (46.97%)cases were the bottom of varicosity in stomach and esophagus; 560 (97.05%)cases were gastritis; 46 (7.97%) cases were gastric ulcer; 52 (9.01%)cases were gastric disease with high pressure of portal vein; 2 (0.35%)cases were stomach cancer; 51(8.84%) cases were duodenal ulcer; 34 (5.89%)cases were duodenitis.HP showed positive in 40.03% of patients: acute hepatitis was 32.43%(24/74),chronic hepatitis 43.97%(102/232), and cirrhosis 38.75%(105/271).Conclusion The research shows that it is common in patients with liver disease for gastric mucosa pathological change and the pathological mucosa changes in upper alimentary tract in patients with liver diseases was not far higher than in documented patients with non- liver disease.[
