超细支气管镜在诊断周围型肺癌中的应用价值

目的探讨超细支气管镜在周围型肺癌诊断中的应用价值。方法对21例怀疑周围型肺癌的患者,先用电子支气管镜行常规检查,未能发现支气管内肿物或者新生物位于上叶尖、后段以下,活检钳难以触及者,再选择行超细支气管镜检查。结果 21例患者中,经活组织病理检查(简称活检)诊断阳性率81%(17/21),联合毛刷诊断阳性率86%(18/21),其中15例为肺癌,肺癌诊断阳性率71%(15/21),2例为炎症,1例为结核。结论非X线引导下采用超细支气管镜检查联合活检可以提高周围型肺癌的诊断阳性率,值得临床开展应用。

Application of ultrafine bronchoscope in diagnosis of peripheral lung cancer

Objective To evaluate the value of ultrafine bronchoscope in diagnosis of lung disease.Methods Ordinary bronchoscope was performed in 21 cases who were suspected of peripheral lung cancer,but were not able to be diagnosed by ordinary bronchoscope,and then ultrafine bronchoscope was performed in them.Results Among 21 patients,the positive rate of ultrafine bronchoscope lung biopsy was 81%,and the positive rate was 86% if combined with brushing biopsy.Fifteen cases were diagnosed as cancer,two cases inflammation and one case tuberculosis.Conclusions No x-ray guided ultrafine bronchoscope and lung biopsy can increase the diagnostic accuracy of peripheral lung cancer,which can be used widely in clinical applications.
