经鼻蝶与经颅入路显微切除垂体腺瘤的临床对比研究

目的:对比垂体瘤两种手术方法的优点及适应证。方法:选用近5年经鼻蝶入路垂体瘤手术的60例病例,及2002年前的经颅手术的30例垂体瘤病例多项作对比研究。结果:两组对比:视力改善率P>0.05,手术时间、出血量、住院日、全切除数、激素改变率P<0.05。结论:经鼻蝶入路垂体瘤切除是一种安全有效的方法,具有手术时间短,出血少,术后恢复快,病人易接受,术后效果好等优点。蝶窦发育差及蝶窦炎症的病人适合开颅手术。

A Clinical Contract Study of Pituitary Adenoma Micro-resedion by Transnasal Transsphenoid Approach and by Transskull Approach

Objective:To contrast advantages and indications of two methods of pituitary adenoma resection:transnasal transsphenoid approach and transskull approach.Methods:Selected 60 cases of pituitary adenoma resected by transnasal transsphenoid approach and 30 cases of pituitary adenoma resected by transskull approach,were selected to do clinical contract study.Results:The comparision of two groups showed P<0.05 in operation time,bleeding,hospitalization days,total resection numbers,hormone change rate and P>0.05 in sight acuity improvement.Conclusion:Pituitary adenoma micro-resection by transnasal transsphenoid sinus is a safe effective method,which has a lot of advantages: short operation time,less bleeding,fast postoperation recovery,acceptable by patients and better effects.
