髂静脉压迫综合征的外科治疗

目的：探讨髂静脉压迫综合征的外科治疗方法。方法：手术治疗髂静脉压迫综合征30例，其中行经腹左髂静脉松解、右髂动脉后置吻合术7例，左髂静脉切开成形、右髂动脉后置吻合术18例，左髂静脉病变段切除、人工血管原位移植术5例；同时行左股静脉瓣膜缩窄及大隐静脉高位结扎剥脱术20例。结果：无手术死亡．患肢肿胀均有不同程度的缓解，原有肢体溃疡愈合或好转。随访21例，随访时间2～36月(平均23月)，效果满意。结论：对髂静脉压迫综合征应采取积极的手术治疗以恢复血流通畅，手术效果良好。

Surgical Treatment of Iliac Vein Compression Syndrome

Objective:To investigate the surgical therapy of iliac venous compression syndrome.Methods:From September 2000 to December 2003,a retrospective analysis of surgical therapy in 30 patients with this syndrom was carried out.Left iliac vein decompression and right iliac artery anastomy postposion(7 cases),left iliac venoplasty and right iliac artery anastomy postosion(18 cases),occlud iliac vein resection and artificial vessel implantation(5 cases) were performed respectively.And 20 cases under went left femoral venous valve shortening and great saphenous vein high ligation and exfoliation simultaneously.Results:No death occurred in our group.Limb swell relieved with varying degrees and ulcers were healed up or mended.The Follow-up in 21 cases ranged from 2 to 36 months(AVR 23 months),and the effects were satisfactory.Conclusion:We should take positive surgical methods in the treatment of iliac vein compression syndrome to resume blood stream,and secure the effect desired.
